1. A 40 year old woman has been admitted with severe abdominal pain and
hematemisis. She has been experiencing similar symptoms over the past year and
is currently on antacids. Endoscopy reveals several lesions in the stomach and
duodenum.
2. A 72 year old gentleman has been having difficulty swallowing for the
past 6 months. He has lost 7 kg which he attributes to not being able to eat
properly. Until recently he was able to tolerate small amounts of liquid but
this is no longer possible. He is in constant pain. On examination, the
patientŐs face and arms look oedematous.
3. A 39 year old gentleman presents to a gastroenterologist with severe
retrosternal pain. On further questioning he admits to drinking heavily since
he lost his job 3 months ago. Endoscopy reveals changes consistent with grade C
of the Los Angeles Classification.
4. A 23 year old medical student has been feeling lethargic lately. She
has also had some unintentional weight loss and a change in her bowel habits. An
FBC showed: Hb 9.7 g/dl, MCV 110 fl, WCC 8x109/l, Plt 295x109/l.
On examination, there are ulcers in the oral mucosa as wells as an itchy rash
on her arms and some bruises. She is referred to a gastroenterologist who
performs an endoscopy and biopsy. The findings are characteristic of her
condition.
5. An 80 year old lady presents to her GP with recurrent severe
epigastric pain which doesnŐt radiate. She describes her stools as ŇtarryÓ.
History reveals that she has had a hystererctomy 40 years ago due to profuse
bleeding. She was diagnosed with rheumatoid arthritis 30 years ago which is
being managed by medication. A CLO test was performed and was negative.
